Hiatal hernia: surgical experience and clinical profile in a national referral hospital, Paraguay 2023

Introduction. Hiatal hernia is a frequent cause of gastric symptoms, such as gastroesophageal reflux, which affects patients’ quality of life and is a common indication for surgical intervention. Objective. To describe the clinical aspects and surgical experience in patients diagnosed with hiatal hernia who underwent surgery at the General Surgery Service of the Instituto de Previsión Social during the period 2018–2023. Materials and methods. Observational, descriptive, retrospective, cross-sectional study that included patients operated on for hiatal hernia during the study period. Demographic data, predominant symptoms, type of hernia, surgical technique used (antireflux procedure), as well as the presence of postoperative complications and recurrences were collected. Results. A total of 18 patients were analyzed, with a mean age of 60 years, 55.5% were female. The most frequent symptoms were epigastric pain (44%), followed by dysphagia (17%) and heartburn (17%). Type I hernia was the most prevalent (59%), followed by type III (31%). Sixty-one percent of patients underwent Floppy-Nissen fundoplication. Postoperative complications were 12%, and 67% did not present recurrences during follow-up. Conclusion. The results suggest that Floppy-Nissen fundoplication is a safe and effective procedure, with low rates of complications and recurrences. These findings contribute to strengthening the clinical and surgical approach to hiatal hernia in the hospital setting.
